     Please enter any comments or questions, directly related to the application process,
   in the area below. They will be e-mailed along with your application and a response
will follow as soon as possible. (Other questions may be submitted by e-mail
to the Dojocho from the Contact Page).
Upon receipt and acceptance of your application, the following will be required:
  • Two passport photos
  • Your martial arts resume
  • Copies of your most recent and/or highest rank promotion Certificates
  • A one-time membership fee of $100.00 U.S.A. currency Checks are to be made payable to A.F.C.

I understand that by becoming a Shuyokai I agree to host individually or collectively one Shuyokan seminar per year in my immediate area. Travel expenses, lodging and transportation for the Dojocho and/or one or more additional assistant instructors, if needed, to and from the seminar(s), independent of the seminar fee, shall be the responsibility of the hosting Shuyokai.

It is also understood that I may register my current rank and/or any future rank promotions for either my students or myself with the Shuyokan at the rates so stated in the Shuyokan Rank Promotion Fee Schedule.

By this form, I hereby agree to the terms stated in this membership application, and understand that failure to comply with them shall result in the termination of my Shuyokai membership.
