Shuyokai Listing Information Form

This form is for submitting the information you would like to have appear in your listing on the Shuyokai page. Before your listing will be made active, please pay your annual $50 listing fee. This fee is due on June 1st of each year and will be considered late if payment is not received by June 10th. Your listing will be removed if payment is not received in a timely manner.
