After completing this form (and pressing the Submit button), the information will be sent to the Shuyokan Dojocho, for approval and processing.

An additional annual administration fee of $50.00 USD is required to have a Shuyokai listing on the Shuyokan web site. By checking this box I indicate my desire for such a listing and I agree to the annual administration fee. I also understand, after sending a written notification, I may cancel at any time prior to the annual renewal deadline. Further, if I do not pay my annual administration fee within 10 days after the annual renewal date, my Shuyokai Listing will be removed.

Other Notes

Upon receipt and acceptance of your application, the following will be required:

  • Two passport photos
  • Your martial arts resume
  • Copies of your most recent and/or highest rank promotion Certificates
  • A one-time membership fee of $100.00 U.S.A. currency. Checks are to be made payable to A.F.C.

I understand by becoming a Shuyokai I agree to host individually or collectively one Shuyokan seminar per year in my immediate area. Travel expenses, lodging and transportation for the Dojocho and/or one or more additional assistant instructors, if needed, to and from the seminar(s), independent of the seminar fee, shall be the responsibility of the hosting Shuyokai.

It is also understood I may register my current rank and/or any future rank promotions for either my students or myself with the Shuyokan at the rates so stated in the Shuyokan Rank Promotion Fee Schedule.

By submitting this form, I hereby agree to the terms stated in this membership application, and understand failure to comply with them shall result in the termination of my Shuyokai membership.
